I inquired as to why my miles were removed and received the following response from UA within 24 hours:
 

UPDATE:
I contacted Chase who informed me in writing that 5,000 points would be posted. However, after 40 days the miles were still not in my mileage-plus account. I spoke to a rep today who told me that I would not be receiving them because Chase had no way of transferring to MP since I closed my Explorer account in November. I suggested that she instead credit me 5,000 ultimate rewards points to my active SP account. After a few minutes on hold, she was happy to oblige and seemed excited that I had come up with such a great solution.
