Good point! Where do they take the fee from? I assume from your Bank account? I got my first statment, and there was no fee taken from the account. And when do I get charged the $25 for the CO card? Also taken from the account?

i pay my tmobile bill every month like that. they dont let me do the same amount twice, so i do 1.01, 1.02....

Your account will be charged for the $25 yearly fee. I was charged in the second month of having it.

for some reason it seems like they only charge the service (6.00) fee starting in the secon statement cycle, proof being that I did not perform any of the "fee-waiving" actions and still did not get charged the fee on the first statement.

I know very little about USA cc, however speaking to some director at chase, he told me about the offer you are talking about, and the card needs to be swiped, physically, otherwise it doesn't count.
This comment was probably completely useless, however, giving it a shot.

Its been proven that it does not need to be swiped. I made all the payments online and got my miles.
