You can get an appointment by fax. I did it a few times. If you need visas for a family of more than 5 people (couple with 3 kids or more), you don't need an appointment.

best way to make an appointment, send them an email Ashrot-Jerusalem@piba.gov.il, with your passport numbers that you need an appointment.

I don't have a scanner. Would you know if I would be able to take pictures of all the documents and upload them into the email? Would they accept that?

nothing, just attached filled form and necessary documents

And for your email address, they simply replied to the email you sent them? Or you had to write it somewhere on the form?

how do i get a birth certificate for an american just born in israel? i know i have to go to the misrad hapnim, but i cant figure out what i need and how to do it. the only thing i dislike more than going to the misrad hapnim is going to the misrad hapnim 2 times.
