I would like to apply for the Ink Plus and or Ink Bold for a family member of mine who is an employee (officer) at a non-profit entity which already banks with chase for 3 years.

What do write when the app asks for annual sales/revenue?
And does the fact of being a non-profit minimize the chances of getting approved?

I've heard conflicting reports if when applying for a CC for a yeshiva if there needs to be a personal SS #, just like when you apply for a biz and enter your SS # instead of an EIN. Anyone have any clarity about this?

There are two places that you need to provide information, one is the personal information, you need to put your social there the second place is the company info that's where some people put their social instead of an ein number.
