If a passenger cannot fly due to medical reasons, is there a way to get the miles back without any fees ?

Within 24 hours of the flight you can sometimes change and be confirmed without a fee.

I'm trying to book AC I class TLV-LGA . It shows on united.com but won't allow me to book the ticket.  When I called, they told me they don't see it. Is that a usual occurrence? Is there any way for me to book the seats through AC? The rep explained that its married segment and she cant book that particular connection.

It's not unusual for UA.com to show phantom availability.

Did you check AC to see if it shows up? (keep in mind it'll be 82.5k + YQ).
