Ksiva is writing or inscription. Chasima is a seal, as in to "seal the deal." Tova is good.

Put them all together and it means literally "a good inscription and seal."

Colloquially we translate it as "May you be inscribed and sealed for good" because that's what the phrase is trying to invoke.

It's a traditionally salutation before Rosh Hashanah which is a day of judgment when the Book of Life is open and people are being written into the book.

Now that Rosh Hashanah is over, the salutation changes. We say "chasima u'gmar chasima tova" which means "a seal and a complete (or final) seal for good."

The way people use it colloquially it just means a good person. As in, "That guy held the door for me. What a tzadddik!"

But like I said, that is more an informal usage. When people are using the word seriously, it is a title of great reverence bestowed upon uniquely holy people.
