I didn't have a visa for the year I was living in Israel because we were home just about every 3 months. We signed up for Harel insurance which we bh didn't really use. The only issue was that Harel doesn't cover maternity and there is some kind of waiting period for 6 months so I wasn't covered while I was pregnant in Israel. I used a private dr anyways for appointments, but blood work, tests, hospital visits etc was not covered at all. It may be worthwhile to get on meuchedet to avoid this issue.

No pre-existing conditions
No maternity

You will also need to use a Harel Doctor

We paid $360 per person for 12months

Not a clue, but I do know that meuchedet is easier to deal with. Not that any Israeli company is easy to deal with.. They have many locations throughout yerushalayim. I needed blood work done once and I found a place near town that accepts Harel and it cost me 2 cab rides and 3+hours. It's not worth the headache.

My insurance from Canada covers me in Israel, but not my wife's (we're currently on separate plans)
We are moving to Israel for one year.

Does anyone know a good insurance plan for a foreigner, in Israel for one year on a work permit?

If you're not gonna do BL, which anyways would be a waste a time for only one year, get Meuchedet. It cost us total 265 sheks a month, it covered pregnancy and was very convenient. When I was in yeshiva I had Harel which I thought was terrific bec it was cheap and it covered visits to Terem 24/7. Instead of making appts by doctors, I would just pop in. Though Harel doesn't cover pregnancy.

ETA: AFAIK, only the Meuchedet on Panim Meirot will allow you to sign up.
