Citi AA signup bonus 50K, 75K, or even 100K??!!

In June, using two browsers, I applied for the Visa and Amex branded cards. Although I applied for both of them, and the page said that I was approved, I ended up receiving TWO Amex branded cards and no Visa card.  I previously had the Visa on the last go-around and cancelled it about 18 months before applying for these cards. 

How can this happen?
Will I get bonus miles on both Amex cards-the first 50K have posted already?
With regard to the $150 statement credit, do refundable tickets work?

I did it on 3 cards in the past 2 months with AA tickets, all posted the day before statement closed.

I pretty much spent the $3000.00 on each to receive the 50,000 sign up bonus on each.
I received in the mail 2 free companion passes (1 for each card) with any purchase of of a full price ticket on AA (which I was not aware was under the promo I signed up for) did anyone else get this?
My question is,
I thought when I signed up I would receive a $150 statement credit per card, but the citi rep said she doesnt see that as part of my sign up promo and that all i have is 100.00 credit if I spend 100.00 on AA. is this true? is there a way to get it?

As far as I know it's $100. That's what I received as well with my 2 AA cards.
And yes everyone gets those companion passes.

There's different offers, you did the 50k $100 Credit companion pass offer after 3k spend, the other offer is 50k $150 credit after 2.5 spend

my companion certs came with a Nov 30 expiration date. I know that its because they stopped issuing the certs, but was anyone able to have them extended or get some  sort of compansation from Citi or Amex?

I haven't done a 2BM for the AA cards before, do I leave the AA FF# blank? Create 1 FF account number for both apps? 2 FF account numbers one for each app?

I put in a new FFN - the same one for each application.
