Only MH was charged in error. If the system/rate desk doesn't add it the silly agents aren't about to.

Probably people were exploiting a loophole to book rev tickets via AA to avoid YQ.

The system didn't. A lot of reps figured it was a mistake, so they manually added it, but it used to fly by all low IQ reps.

I kinda agree with him.
Having their twitter team basically agree to the new charges, and them not simply spewing info they're uncertain about (especially info this sensitive) implies that s/t did go down in between.

AA used to book revenue tickets without pricing the YQ automatically. Most reps used to realize that themselves, but with the more clueless ones, you were able to price and ticket without YQ.
