LPT: Cut the buns from the top down, not on the side. A bit weird but works well.

Big companies that don't take security seriously.

American Express:
1) Does not offer 2 factor authentication.
2) No security authentication when logging in from new device (or location).
3) Passwords are not case-sensitive.

And then for that answer, there's a minimum of 8 characters, when your answer only has 4 or 5.

Alternatively questions that, if you have been compromised are easy to come by - or questions that could easily compromise you're information further.

"What street did you grow up on?" If someone has my password without my knowledge, they can easily figure this out too.
"What is your mothers maiden name?" Really? I thought this was supposed to protect me, not make me more susceptible to having my identity stolen if this account gets compromised.
