As with all vendors, you need to price compare and negotiate. Personally I would recommend Banquest for great cs.

Difficult to know exactly. As I said you got to negotiate and get multiple quotes. Make sure they explain everything and all fees to you (which banquest did better than others) and ask what your max effective rate will be. See if you can get it under 2.8%

I don't believe so. When I was in Israel recently there were visa/mc/sometimes amex stickers at registers but never saw a discover one
