Cholov Yisroel:
If you go onto Kosher Panama Website you can search for Cholov Yisroel by entering Jalav Israel in the search, just be aware if the site lists Jalav Israel and Lacteo in the description, the establishment may have both CY and non-CY so be aware and double check with the staff.
SuperKosher also had a nice selection of CY breakfast/snack/frozen items available.

There is a local chalav yisroel that isn't up to everyone's standards. It isn't up to Chabad as they use a lower kasherimg temperature than the alter rebbe holds.
This is what local chabad down there told me. Don't hold me to any of it please.


Synagogue:

The Jewish community in Panama is mainly in Panama City in the Punta Paitilla area.
You must bring your passport with you to enter the shuls. You can bring your passport over during the week if you are not staying in the eruv. They will make a copy of your passport.

Sinagoga Beth El – Orthodox, Ashkenazi. After Shabbat morning services, they have a full sit down Kiddush lunch.
They also serve breakfast after weekday shacharit.

Any particular reason you're not considering the Residence Inn? Been discussed here quite a bit.
From what I read it seems that people went there because it was shabbos friendly and walking distance to Shul. Since I’m not being there over shabbos didn’t think it was necessary.
Felt the options that I listed were nicer, but I may be wrong.

The Residence Inn and Eshel are both walking distance to shul and the supermarket and some restaurants.  So you save travel time and bother.

Going for 3 days without kids.
What's a must?
We would love to do Monkey Islands, San Blas Islands, Miraflores-Panama Canal. It's 3 full days and would love to squeeze in as much as possible. Is there anything else that's not to be missed?

Regarding Hotels we would like something nice and they all seem like an uber away from the shul.
Options are The Waldorf, JW Marriot, The W and La Compania.

Any suggestions and advice would be greatly appreciated!
Itinerary looks good. We preferred the Hilton because it is just across the road from the ocean so the view is better than from the Waldorf and the rooms were nice and larger than the Marriott.

La Compania is a stunning five star renovated historic building (covenent converted to hotel) from the early 1700's in the center of Cosco Viejo (the Old Quarter), Imo, I would highly recomend staying there if you appreaciate history and architechure. The garden was beautifully redone as well. The others you mentioned are standard nice hotels but closer to minyan and kosher food.

Went about 2 years ago. We stayed at the Residence Inn. I really enjoyed it. Though I did try 3 different hotels while I was there. 
  Switched to the Residence in for shabbos and stayed for the remaining 5 nights. They gave us a corner room which had stunning views. Downside was there was no porch.  But nice little kitchen for shabbos and kitchen sink.
 Then even botched up on the nights and gifted us 3 free nights. In short I had booked 2 nights with marriot points. (There was an offer to book 4 nights get the 5th night free.) I liked the hotel and decided to extend the stay another 3 nights. So called to add 2 more days and see if they can manually add the 1 free night. As I now had 4 consecutive nights booked, though not in one booking. So all I wanted was the extra day added. But sure enough the answer was they couldn't add the single free night as it needs to be one booking, but to my surprise the agent added all 3 nights I wanted for free. (May be the reason why I liked the hotel, so yes a bit biased. ;-)

Note, its pretty high up (2000 ft per my watch), so much cooler than reg Panama city and the ride there is pretty hilly.
