The driver should pay the toll and it gets added to your Uber bill. You don't give cash to the driver to pay the toll. The toll amount added to your Uber bill might not be the same amount the driver paid. For example, the driver could have an E-ZPass or a specific E-ZPass plan that gives him a better deal.
If the driver takes you somewhere where he has to pay a toll to return to his area, you could be charged for that toll too.

I've had it that the driver pays tolls and it's added to the bill.

Not sure if the estimate included it, but my estimate on a route with tolls was pretty on target.

Does anyone know if uber and/or lyft charge extra just to go in to lawerence? I know regular taxies charge more if u go in to long island as opposed to staying on the queens side of the hwy

Doesn't the page say it all? It's for employee who get commuter benefits from their employee. However I would advise against using such card and instead getting a refund from the employer directly if possible since "When paying with your commuter benefits prepaid card, you will always be connected with a driver-partner whose vehicle seats 6 or more." That means the driver can pick up/drop off at least 4 people, you'll be much much quicker taking the subway or Via in Manhattan as UberPool's algorithm there is very bad.
