Can anyone clarify whether there is any downside to calling AA before the flight to add an infant (on lap) to an domestic Avios reservation?  (Is it free in the airport, but not free on the phone?)

Is it any different for adding an infant (on lap) to a paid AA reservation?

No downsside to calling AA to add it. If you call BA they will likely say there's a charge.
