Well first things first.... go to a dr and get your updated prescription. Next - IMO - you don't usually have to write anything by cyl, axis unless the dr. clearly writes something about it. I ordered for three people on Zennis (including myself...) and for only one person, they had numbers by cyl/axis... we left ours blank and so far we're enjoying the glasses.
