Suggestion: Have someone else have the password (or half and half with a spouse) and do the following:
Lock installing apps + remove browser + add apps you need (possible rules: no games, no sports, no entertainment, etc.).
[Occasionally an app will have a built in browser/or need to access a certain website - just go back in to settings (with password) and add that website].
There is time restrictions and more features too.
(May not be enough for teenagers - consult your own advisers - this solution sure beats doing nothing and there is no excuse)

Seems that Qustodio is not blocking or tracking anything on my phone when using Chrome incognito. Is there a way to change that?
There's a setting for incognito which shuts the browser down if you use incognito, but it doesn't work well.

The way to change that is to use Netspark or Livigent instead, or at least a filtered browser such as Ranger, Mobicip, or Kiddoware Kids Safe and block the stock and Chrome browsers with an app lock.

Qustudio itself has an app lock that you can use to lock the play store (and Amazon app store if you have it).

The one issue with qustodio,
 the person with the password sees every website that you used, how much time you spend in each app etc.

I need a filter that doesn't give the person with the password ALL that info. One that just filters, and gives the info of questionable sites visited.

True, but meshimer has it highly fine-tuned for our crowd's needs.
With Mishimer, Ok Google doesn't work. I use it all day ( and night ) for reminders.

Not as far as I know.
In theory, turning the incognito setting on should work, but it doesn't detect accurately.

It all depends on how motivated the device holder is to access bad sites.
There are ways of installing even if Play Store is locked.
