$40 from T-Mobile

If you have T-Mobile, they are currently offering a $40 flat refund to customers to settle charges related  to the dispute over unauthorized charges for third-party Premium SMS (Text Message) services.  I received an email about this, so it may be a targeted offer, although I do not recall being billed for any of those 'premium' messages.
