1) Can I combine a anniversary night, together with additional points, to use the certificate at a higher category then just four ? 2) Anybody have suggestions where I can get a good value from the category 1-4 certificate ?
1) don't think so
2) how about at least narrowing down to which hemisphere you ate looking for

Another good tip is to google/use Hyatt's website to see where the Cat 1-4s are. If you're limiting yourself to 4-5 hours, then you're basically saying North/Central America, which should knock out many of the 1-4s to begin with. Find some that are in locations that interest you and then ask/research to see if the properties are actually nice/near activities in those cities.

Does the cash part of c+p reservations need to be paid at the hotel, or can it be paid on the phone while making the reservation?
