This is just wording the question badly to make it sound more 'riddle-y'. The actual question is basically name a contronym (or autantonym). And there are many answers, though cleave is certainly a prominent example.

Actually 'it' has 2 letters
Therefore, I'd word it differently to make it a bit harder, more "riddley", and throw people off.

What has 4 letters, sometimes 9, but never 4.   (since 'but will never have anything other than 3.  IOW, play off the 'but', not the 'never'!

Well if he's dying due to the weather have him warm up in the car. Then talk to your friend with the window down or go outside the car.

Yes,  amazingly most people I asked this question to said ten cents.

This is a psychology experiment - I don't believe it is a Twersky/Kahneman experiement, but I believe they used it in their seminal System 1/System 2 paper - the basis for the Book "Thinking Fast and Slow"

Another alternative is a natural father whose son was adopted by the other.

The simplest answer is that the Doctor is his mother. People "of a certain age" fail this, because they aren't around enough female doctors to think that way.
