Another alternative is a natural father whose son was adopted by the other.

He wrote his son first, and then the son. But in any event, it seems like @TimT was right. Though if you got it on time before the edit, you can see that he too wasn't thinking straight at first. Pun intended.

...People "of a certain age" fail this, because they aren't around enough female doctors to think that way.
I'm not sure this is the reason. I think the way the question is framed plays a big role as well. There are 8 words in the riddle that are gender specific, and all refer to males.

what would you do if:
on a cold winter rainy -terrible weather- day, you drive down the street with your car passing by a bus shelter you see 2 people sitting there
1) an old and sick person who looks like he is about to die due to the -terrible weather- and needs a hospital ASAP.
2) a person from a different country you've been looking for a very a long time to speak with and is never here and now you need to grab the opportunity.
but the only problem is that in your car you have place "only" for 1 more person there is 'absolutely' no way another person could get into your car.

whom would you take?
Call an ambulance to take the sick person to the hopital

Let the friend take the sick person to the hospital and then come pick you up

If this is the answer, the riddle has holes.

According to this, if the reason he can't leave the person from another country is because that person won't have enough time, then letting him drive the old man won't solve anything. And if he has plenty of time, that person could also wait at the bus stop while he drives the old man himself.

This riddle is missing the third factor which is the third person waiting is a friend who once saved his life. And therefore the answer is let friend take sick guy to hospital and u get to spend the bus ride catching up with the overseas guy
