He’s born as part of a set of triplets, he’s the oldest, but not ready to get a bris right away. In the meantime the other 2 siblings get a bris and die. Then the oldest triplet becomes healthy and is patur.

2 scenarios where he’s always been healthy -

1. The other siblings simply had a bris first. The minhag is to do the oldest first, but I didn’t see a Halacha against it. But the better answer is -

2. The oldest son was born Ben hashmashot Friday night, with a delayed bris until Sunday, while the others were born on Shabbos. Hence the other siblings had an earlier bris.

if the 2 siblings before him died because of the bris Milah, it creates chazaka for that family to die by bris and therefore exempts the rest of the brothers in that family from needing to do bris
