LH can be 17k - YMMV.
Call LH and book using LH miles.
1)  If I do not have a Lufthansa card, what is the best way to take advantage of their 17k fare?  Which card is best to transfer into my miles&more account?

2)  Also, my miles&more account only has a temporary number.  CSR said I need to have miles in there in order to get a permanent number.  I can't look online to book until I get a permanent number.  Ideas?

If you are looking to book EWR-LAX then search UA.com for their availability. Otherwise you can call LH for them to search.

You didn't specify which class, but I'd guess CX for 30k in Y or 50k in J with Alaska, or 67.5k in F with AA, are probably the cheapest options.

EWR-TLV in J with UR points?
80k with United miles for no fuel surcharges. 57.5k with Singapore miles which have fuel surcharges on carriers other than United (fuel surcharges when flying LOT Polish are significantly lower than other airlines with fuel surcharges). 70k and no fuel surcharges with BA miles if you find availability with Air Berlin.
