Its possible to do by using router and formatting software to DD WRT with special configurations. Works well.

Care to elaborate and /or link?

OMG!:
My Optimum extender is all set, but I like to boost the signal a bit.
Can someone recommend a Yagi Antenna that works with 5GHZ (unless a 2.4GHZ would also work to catch the optimum signal)?

Sorry Just saw this.

You need to buy a dual router that can be reconfigured with software called DD WRT. One antena will be used to receive Optimum and the other to redistribute. U still need a optimum log in and smart devices might not work.

Would this extender work? What needs to be done?

No.
You need to install DD-WRT which can only be installed on a regular router. Once you install DD-WRT follow these instructions: https://www.overclockers.com/forums/showthread.php/747884-How-to-use-an-Optimum-WiFi-hot-spot-with-multiple-computers
