Will elal and eta IL be okay with that?
If they are coming to IL to live, it is easier to have the same last name re: any IL documentation (bank accounts, birth certificates, etc). I had a fun time with kids that had the wrong last name on IL birth certificates.

Not sure there's anything to do about it - it depends on Misrad Hapnim policy at the time and how much of a power-trip that the fellow behind the desk is on. Lemaaseh the consulate/embassy is used to it, and they'll put whatever last name you want on the US documentation.
Still - keep every bit of documentation regarding the birth - ten years later, I needed the tissue-paper temporary certificate to prove that I was the father.

Change your wifes name on her passport?
Don't know what he went through, but all my kids were the same thing on the original, it's not the end of the world, you update it when you go into misrad hapnim to get the final copy with the updated first name
