+1 on the fee-free withdrawals but I don't believe you can deposit.

Does amex officially allow one to have canadian and us accounts?

I don't have suggestions off the top of my head. What's the spend requrement? Does the card charge foreign transation fees?

They charge 2.5% transaction fee. It might just be worth it for me to open a Canadian Paypal account and send someone money. I have seen some posts saying PayPal doesn't allow this but I'm not sure if that's correct. Anybody try doing that in Canada?
