Best International Business Card?

I am looking for the best International Business Card.
Features Required: No Forex Fees, Able to take out $5k from ATM internationally, and possibly insurance.

I have tried the Card comparison wiki and the links were not helping. I have checked around the forums but cannot find the information I am looking for.

Being that you want to take out money, you are looking for a debit card. That doesn't really go along with car rental insurance
