Things you MUST submit with your rebate:
1. Fill out the online rebate form from the link that's on the item page (or on the forum whichever is easier or posted)
Make sure you are within the limit of that rebate offer (number)!.
a. Make sure you put the correct order number.
b. Make sure you put the right order date.
c. Note: you must give the BILLING address (not the shipping address).
2. Print out the form which you submitted thru their website (you can do so any time by entering the status page with your confirmation number).
3. Attach the entire UPC code cut out (cut out the whole UPC together with the cardboard, don't just peal off a sticker, I like to stick it on the form with a piece of tape where is has the picture of a UPC code).

SIGN the form AND make sure the date is correct. (This is the most important part, other proof you have a second chance to submit).
5. Submit a copy of the INVOICE or PACKING SLIP
"NOT THE SHIPPING OR ORDER CONFIRMATION EMAIL!" (You can print out an invoice thru your TD online account or by calling them up they can email you one.
6. Put the correct address on the envelope, use the address from the rebate form (this address contains the offer number), you can just cut it out and tape it on the envelope, make sure it's within the time frame (30 days from order date usually).
7. Put a stamp on the envelope and drop it in to a mailbox.
