Missing rebate / UPC copy?

I never received the rebate for the LG HBM-235 (http://www.dansdeals.com/archives/11695)

They claim they haven’t received my submission. I googled a little and found out that they did this to many people.

They still gave me the option to resubmit by fax. Unfortunately I didn't keep the materials. Does anyone have a copy of that UPC code?

This is precisely why I view rebates as a bonus, I don't rely on them, if I get them I'm lucky.
