Okay, just got off the phone with a very high-pressure salesman from CapitalOne, and here's what he said:
1) It's 100k miles earned with any one single credit card's program, not a combination of cards and/or programs. Has to be only 1 card crediting to 1 program
2) No time limit on the earnings - can be 100k miles earned in the past year or five years together...it doesn't matter when the miles were earned for matching purposes
3) Can submit a CC statement or a airline program statement, as long as it shows that X amount of miles were earned or deposited...this info is sent along with the card upon approval.
4) Upon approval, 100k miles are instantly reserved and set aside for you. You then have 90 days to submit your proof of miles and spend $1000 for the full mileage amount. The 10k bonus is not part of this process and is earned within 1-2 billing cycles, along with the matched miles.
