There are some places where foreign cards are not accepted. It's happened to me on gasoline, for example.

Yeah for gas you have to go inside. Then they will swipe it for and it will work.

In general, it's much easier to negotiate when paying cash.

That's true especially in the Shuk and certain places. However if your just looking to some restaurants, rent a car pretty much do basic stuff I would think it makes the most sense to stick with a CC.

Will the sapphire give 2x for travel in taxis? What will be cheapest for me? Exchanging American cash for shekels at airport or just using sapphire? Also where's the best place do get shkalim for American dollars?

Don't change dollars for cash at the airport.  I've had taxis categorized as transport/travel with Barclays.  Any change in Yerushalaim should give you 3 agarot below the mid market rate.

The airport is the worst place to change money in terms of rates.
Any money changer in Yerushalaim should give you 3 agarot below the mid market rate.
Just make sure you know the mid-market rate at the time you walk in there. Also bring hundred-dollar bills or at least 50s for the best deal. Money changers here tend to give lower rates for quantities of smaller bills because they have a hard time getting rid of them. (I was in the business until very recently.)

Even worse,they add it to the total so you don't even get reimbursed from your bank

No idea. Go to the foreign banking department in that branch and attempt.
I got a text from leumi saying that I have to come in the branch. I just tried my card in their machine and it doesn't work. What do they want in the branch? I have a couple of thousand shek in there.
