Macys rebates - black Friday

There was a deal posted on DansDeals around black Friday time to get small appliances from Macys with a mail in rebate I.e. Pyrex containers, A Bella panini maker etc. But when I look at the form for the rebates I don't see any of these items to check off. Anyone know how to do these rebates?

this isn't the one from black Friday, this is the  current one. the problem is that once the new rebate gets released the old one gets lost someplace in cyberspace. ive already spoke to several supervisors who couldn't locate the form for me. ended up returning the whole lot

I believe you are supposed to mail in the "Original UPC code cut from the product packaging" with the rebates.

If you got a few items like let's say 3 do I have to do seperate rebate forms or could I do all 3 together?

I'm not familiar with this rebate but it depends how they set it up, you need to read the terms
