Black Friday Kohls rebates

There was a deal posted on DansDeals around black Friday time to get small appliances from Kohls with a mail in rebate I.e. Immersion blender, A Bella panini maker etc. But when I look at the form for the rebates I don't see any of these items to check off, it seems like that link is now linked to a new rebate form. Does anyone know where to find the form for these rebates?

i know this is an old thread but havent found a more recent one for this topic. i submitted two rebates for two different products in same envelope and only received one rebate. was i supposed to mail separately or is this their error?

Are you positive the rebate amount was only for one and not both? I personally sent them separately, and got 2 cards.

yes. was a $12 rebate and got one $12 card

Call them. I only got confirmation on one but they found the 2nd one when I called in.
