$300 per item, 1K per card per year. So if you have an SPG card and a PRG card you'll only have 2k of total RP coverage, regardless of how many AUs you have on each card.

If you are unsure if an item qualifies for RP, then instead of asking for opinions here (slightly better than asking medical advice on facebook mind you), PUTPAC and ask the RP reps. You don't need to give your name or card info, just say you have an item or you will be buying an item and the store has a crappy return policy and want to make sure if there is a problem you can return it.

Under "Item cost with taxes (USD) excluding shipping fees" do I put in the full amount that I paid and hope to receive $300
or insert that the cost was $300 since that is the max they give?

I do not see why that would matter. As long as you had not paid for part of it with another payment method you should be fine (provided the item keeps to the terms).

Should be fine. They may ask for explanation of the refund but otherwise will be fine
If I pay for part of an item with an Amazon promotional credit (the $20 Amazon Moments store credit in this case) or a gift card, does that void any return protection on this order?
If it's a gift card it's not eligible (terms say must be paid entirely with the card). If the moments credit shows on order documentation as a promotional credit or coupon it's fine (speaking from first hand experience)

The Moments credit shows on the receipts as "American Express Credit". Thoughts?

Was it one claim for both or 2 seperate claim submissions?
I find the low dollar claims (in the area of under 50) tend to get approved right away without even needing Receipt or form filled out

I have had recent $300ish claims, one took a while and the other took 2 days. I think it's random.

Has anyone had to send back recently? Curious if they have nixed this due to COVID. I noticed that the language about whether you can keep the item or not is no longer in the decision document.
