You go to United's website and open a new account. It's that simple. You must enter a new address or else they'll say you already have an existing account (unless they recently changed their policy).

You could put in any address and change it once the account is set up (they won't let you merge accounts unless all the info matches).

I used my wife's maiden name at my home address. Can I apply now under my name and same address without them merging ff accounts.?

I would assume so.

Within 90 days, yes.

How long ago?

I'd say around 2-3 years ago. This was public knowledge then. Dan even recommended the address trick, IIRC. Was not aware they now let you open two accounts under the same address. Could be that is what's contributing to the few recent reports we had about them contacting United and consolidating the accounts on your behalf (thank you Chase! ) and not giving you the sign-up bonus.
