For theft they'd probably require a police report.

I'm gonna submit my $489 repair quote to Amex today and see what happens

I mean what I said. It might depend on the item value and they might just accept an affidavit.

For a lost item they'd probably require proof of purchase and a signed affidavit, but I have no experience.

Is one's amex purchase protection only activated as long as that specific card is open?  I.e. if I bought an appliance with an amex plat that has since been cancelled, am i still covered?

I know purchase protection covers one additional year above the manufacturers warranty.  If I buy an extended warranty, say for 5 years, with amex plat, am i covered now for 6 years?

Whatever she actually disbursed for the phone up to $300 IIRC (unless it's a Plat IIRC).

Not sure about your specific question, but regarding extended warranty you will

You thought wrong.  Any active AMEX with warranty coverage is good enough.

Made some purchases at Crate & Barrel on an SPG Amex and when I was moving, the movers thought it was junk and threw it out. Is there any protection from AMEX for that? The purchases were made 2 weeks ago

Maybe you misunderstood me, but I wasn't suggesting you commit insurance fraud 

I was asking if you can make a good faith argument that they were stolen. If you cannot, you cannot.
