Say you live in NY and are booking a booking JFK-LHR-JFK.
Being that a ticket is valid for a year and a stopover is allowed in the gateway city (in this case JFK), you can book an extra leg onto Hawaii for any date within the year.
Changes are free, so subject to availability, you can change it to the date that fits you.
To return home you can book a one way ticket or if you are planning another award on AA you can do the same in reverse.
And you have yourself a free ticket to Hawaii.
Obviously, the destination can be changed to anywhere in America, Hawaii is only an example.

So it really is only a free one way per international RT? Is this only on AA?

Assuming you mean One-Way award and not OneWorld award you can only stop at the North American gateway.

Yes.
Of course on a OneWorld award you can have as many stops as you want.
And on a JFK-TLV-JFK award you can add on JFK-HNL for free for a later date that you use, change the date for free on, or throw away.

AA/Partner Award Chart: 1 stopover at the NA gateway city.
OneWorld Chart: Up to 16 stopovers.
