The general practice for an affidavit in NY is to include the State and County where it is sworn/affirmed to. If they want an affidavit, you should include that info. If they want affidavit then it should also say something to the effect that you are swearing/affirming the truth of what you have said. If they just want notary (and no swearing) then it's just to confirm it's your signature and all you should need is date and notary sig/stamp
