MV-104 - Accident with Rental Car

OK, so I rented a car in NY and got sideswiped while parked  over a weekend :-\

I have the Chase Sapphire Reserve so I should be covered but I am not sure how to take care of the police report.

I went into the precinct and they told me I could either go back to the scene of the accident and call 911 or fill out the form MV-104 and mail it in. Having a flight to catch I took the form.

Now I am having some difficulty completing the form... anyone have experience on this?

Here are my questions:

  • Date, do I date the accident as the date I found it or leave it blank?
  • Being that my car was unoccupied, I leave driver 1 blank, correct?
  • How do I complete the registrant info for vehicle 1? Do I contact the rental company for all that?
  • Insurance, how do I fill this part out?
  • I have 10 days to file the report, does that go by postmark or receipt? Today is the 10th day!

If anyone can give any insight I would greatly appreciate it.
