5% categories for fourth quarter of 2017:
Dept. stores and Walmart

There are three possibilities for how many points are earned. In the following examples, the effective rates are assuming you cash out the points for 1 cent each, but if you can transfer them to Sapphire Preferred or Ink, the effective rate could be much higher.
  • Cardholders without a Chase checking account
1 base point per dollar for all spending. Additional 4 bonus points per dollar for spending up to $1500 in quarterly categories.
Example 1: Spend $1 on a parking meter. Earn 1 point, an effective rate of 1%.
Example 2: Spend $10 to refill a Metrocard in 2016 Q1. Earn 50 points, an effective rate of 5%.
Example 3: Spend $100 to buy a Long Island Rail Road Peak Ten-Trip ticket between zones 1 and 3 in 2016 Q1. Earn 500 points, an effective rate of 5%.
    Example 4: Spend $1500 at 7-Eleven in 2016 Q1. Earn 7500 points, an effective rate of 5%.
  • Cardholders with a Chase checking account in older program
1 base point per dollar plus 10% of the base points plus 10 points per transaction for all spending. Additional 4 bonus points per dollar for spending up to $1500 in quarterly categories.
Example 1: Spend $1 on a parking meter. Earn 11.1 points, an effective rate of 11.1%.
Example 2: Spend $10 to refill a Metrocard in 2016 Q1. Earn 61 points, an effective rate of 6.1%.
Example 3: Spend $100 to buy a Long Island Rail Road Peak Ten-Trip ticket between zones 1 and 3 in 2016 Q1. Earn 520 points, an effective rate of 5.2%.
    Example 4: Spend $1500 at 7-Eleven in 2016 Q1. Earn 7660 points, an effective rate of 5.11%.
  • Cardholders with a Chase checking account in newer program
1 base point per dollar for all spending. Additional 4 bonus points per dollar for spending up to $1500 in quarterly categories. Annual bonus of 10% of the total of base and bonus points (until February 2016).
Example 1: Spend $1 on a parking meter. Earn 1 point on the next statement and .1 points at year's end, an effective rate of 1.1%.
Example 2: Spend $10 to refill a Metrocard in 2016 Q1. Earn 50 points on the next statement and 5 points at year's end, an effective rate of 5.5%.
Example 3: Spend $100 to buy a Long Island Rail Road Peak Ten-Trip ticket between zones 1 and 3 in 2016 Q1. Earn 500 points on the next statement and 50 points at year's end, an effective rate of 5.5%.
Example 4: Spend $1500 at 7-Eleven in 2016 Q1. Earn 7500 points on the next statement and 750 points at year's end, an effective rate of 5.5%.

I emailed them your "template" and got rejected. Asked for reconsideration family got it etc. and got this response

We will gladly match the direct marketing offer you
received once you provide us the invitation code which is
listed on the application portion of the document. This
number is 12 digits in length. Once we receive the
information, we will look up the invite code validity. If
applicable, we will adjust the rewards as requested.
First use premium offers can be matched within the first
90 days of opening the account.

At this time, we do not have an online offer for 30,000
first use reward points. Therefore, we are unable to
honor your request to match. However, I show your account
is eligible to earn 10,000 reward points after $500.00 in
purchases is made within the first 93 days of opening.

If you have any further questions, please reply using the
Secure Message Center.

True, as my wife has one as well.

My current strategy is :
-$25+ purchases (including car rentals due to primary CDW), and foreign purchases, excluding dining, airfare, hotels, telecom, cable, office supplies, 5% category, gas, Open Savings Partners, and Gap Inc purchases on United Club.
-Purchases under $25 and 5% categories on Freedom.
-Dining, airfare, and hotels (including foreign purchases in those categories) over $10, and car rental in Israel goes on Sapphire Preferred WMC.
-Purchases at SPG hotels as well as purchases that I want warranty, return, dispute, or purchase protection on goes on Starwood.
-Open Savings purchases go on Starwood Business.
-Hyatt purchases on Hyatt.
-Telecom, cable, office supplies (including gift cards for Amazon, etc), and gas on Ink Classic.
-Gap Inc purchases go on Banana Lifetime Luxe.

I don't have an AMEX PRG right now, but I'll probably shift airfare and groceries there when I get my next one.
Which card would you use for gas, the Amex PRG -2points/$ or the BofA -3% cash back, (for someone with no Ink)?

Right now (July-Sep) I would use the Freedom for the 5% cashback. After Sep use the PRG.

Reply 'but my father got it without an invitation code'
Persistence paid off :) Thanks all for the help

I would be happy to add an additional 20,000 points to
your account ending in XXXX if the $500 spending
requirement is met within the first 90 days the account is
open. If you meet the spending criteria within the
required time period, please send us a follow up e-mail
and I or one of my colleagues will be happy to add the
additional points.

If you have any further questions, please reply using the
Secure Message Center.

Sincerely,

Dawn Stuhr

Yes, once it shows up when you log in to chase you can send a SM.
