Just put him as an additional guest on the reservation, 99.9% of the time its no issue.

However, when he shows up in his tux and the bride in her dress, it will be very obvious to everyone at the hotel that the account holder will not be joining them. Is this something I should be worried about? Does anyone have experience with this?

There were no issues.

Just get the grooms name on the reservation

If you're worried you can always do what Dan suggested.
