A bank statement isn't enough?
A bank statement wouldn't be sufficient. When I was FRed by Amex they requested that I fill out the 4506 form and a letter from my bank confirming my home address. I called up Chase, Citi bank and Ally bank and neither one of them was willing to help me, so I went to A chase branch and a Citi branch and they both told me that they cannot confirm my home address because they don't know that I live at the address they have on file. I then contacted BankDirect who were willing to write a letter confirming the home address that they have on file. Ultimately 3 months later after Amex received my tax returns they tried to verify the authenticity of the BankDirect letter, however, the supervisor that wrote the letter had resigned so they needed a new letter. I got another letter from BankDirect and somehow solved this issue. I could tell you that it wasn't fun at all, and that if you know a knowledgeable banker, which is hard to find, then you'll be able to get this letter. Alternatively, if you have a BankDirect account just send them a secure message requesting it. Good luck you'll need it!
