I am speculating that cancelled orders might have selected no-rush shipping, while shipped orders might have selected 2 day (or 1-day if it was a free option).

Also, might it be the difference of selecting an unregistered Kindle vs leaving the order as the default which is a kindle registered to the ordering account (which presumably takes a drop longer to ship)?

mine shipped  with 2 day shipping and i did not unregister from my account until after it shipped. so i guess you might be onto something

Mine shipped; 2 day shipping and did not unregister.  When I had tried to place the order as a gift, it wouldn't give me the $40 discount.

I am now thinking that nothing is done on the actual Kindle to register it, they just associate the serial # with your account.
