It's actually possible to get an additional card straight from the website. Under Services, select Request Replacement Card.
Note that you need to wait 30 days from the day you signed up for the card before this will work. Otherwise it will tell you that you need to wait 30 days before requesting another card.

possibly. i was shut down 3 years ago (not for ms). i applied now for the miles and more card, was auto-rejected, sent in a letter, and got approved.
