Chabad's enterprise code will work for the underage fee.  Who are you renting with?

I just heard that a friend walked into Avis Tel Aviv with a letter from MAstercard saying their card covered CDW/TP (which I also have) and they wouldn't let her out with a car until she purchased the insurance from them. Anybody have this experience? It's a #300 difference on my next stay/rental. Thanks.

Are you sure the "insurance" in question was not actually LI (liability insurance, which is the fourth insurance box on Avis Israel contracts)? LI is mandatory when declining CDW/TP in Israel.
