US Air Shut Down

Has anyone ever had their US Airways account shut down, tickets cancelled and all mileage confiscating for being accused of selling miles when this was not the case?  If so, can anyone provide guidance as to the most effective channels to communicate our case? Responding to the e-mail sent and calling customer service has gotten us nowhere.

"Tried" to do a mitzvah and book my rabbi's daughter ticket and got shut down.

Did you take payment for the flight? Did you ever share miles into the account?

Accounts with miles transfers (100% bonuses and the like) have been reported shut down.
