Highly recommend: The Allure hotel. Great prices, great accomodations. Guests said Shabbos there was super easy (the staff opened the rooms for them), and it's right near Chabad and the Jewish Quarter.

Places to see:
Jewish Quarter- The Old Cemetery (Maharal and Kli Yakar), The AltNeu (Maharal Shul), the Pinkas Shul (which has all the Czech Holocaust victims' names hand written on its walls), the Maisel Synagogue (which houses many artifacts of the Jewish history in Prague), and the Spanish Synagogue, museum, (can daven micha/maariv at oldnew shul and get free entry). You must buy a ticket for one time entry to each site at the Jewish Museum, at Maiselova 15.
A warning about the Jewish Quarter: as opposed to many cemeteries and shuls around Europe, which tend to be very quiet and free of crowds, the Prague Jewish Quarter is a huge tourist attraction due to its preserved state and proximity to other tourist attractions. Therefore, don't expect to get some quiet davening in at the Maharal during tourist season...
Old Town Square- municipal house, powder tower, astronomical clock (hourly show), wenclesas square,
Charles bridge- the most famous bridge in Prage. Has a statue of Yoshka with the words of Kedusha emblazoned on it
Petrin Tower-an Eiffel tower style cast iron viewing deck overlooking Prague on the other side of the river.
Prague Castle-The largest castle in the world, with buildings and architecture from many kingdoms and eras in central Europe. Hitler spent a night there.
The Jerusalem Synagogue- An absolutely gorgeous shul (some say the nicest in Europe). They have davening Shabbos day. Costs about $8 to tour, but if you have the Jewish Quarter tour ticket (which covers the shuls there, the Chevra Kadisha building, and the cemetery) then it's half price. Noda B'Yehuda Kever (at zizkov TV tower in prague 3, its an easy 4 stops on the metro from old town, open every day till 4 friday till 2),

45 min night boat tour on river,

Terezin (Tereizishtadt) concentration camp -  The concentration camp the Nazis used to pretend the Jews had good conditions. Really really captivating. A hidden shul was discovered there. Drive or hr bus from train station. 

Holesav- A two hour drive. Kever.  of shach ,

Kolin, Nickolsburg, Brinlitz, Boscovitz-All towns with significant Jewish history within a couple hours' drive from Prague.

Another highly recommended tour guide/operater is Yaakov Schwab (recommended by R' Paysach Krohn) Whatsapp: +420 604 857 554 . He can arrange drivers and tour guides throughout the area, as well as drivers to Austria, Bratislava, and Budapest. Great guy!

King David hotel has a tour everyday to the major places.

Kosher list

1) always keep a few krona change in your pockets for bathroom breaks. It costs about $0.15-$0.25 USD (in Krona) per visit no one will let you in for free. (besided the chabad restaurants).

2) You can rent boats to go on the Vlatlava river, not sure the cost. You can also rent "City Bikes" in the old town square for 300 Czech krona. (About $12) for 2 hours & every additional hour its 50 krona which is about $2 USD.

3) All Jewish attractions close at 6pm (or 4 in winter).

4) in the summer, the shuls all make early Shabbos, including Chabdad

It was fine, but doesnt compare to the Intercontinental when it comes to convenience

I took the train, its long but comfortable and definitely worth the few dollars to upgrade to First Class. I ordered to go food from the Chabad restaurant and they packed it well.

I made a reservation online (it was over a year ago so I do not remember all the details)
Timing I believe was 6-7 hours. Price was 50 euro for First Class (again if I am remembering correctly
