Phone just any Sprint (not prepaid) phone, SIM I would say eBay is your best bet

Try to open a support ticket or chat
Edit: i jut noticed that Amazon is selling Ting SIM cards https://amazon.com/Ting-GSM-SIM-Universal-Nationwide/dp/B013LKL5IQ/ this may be the best option, do research, I'm not sure

3) After that, you will have Free/month: 6,000 talk, text, MMS, and MB (full-speed 4G LTE data) going forward.
