Aeroplan - Flat Fee per booking $50 (coach), $100 (Business), $125 (First) All the these prices are CAD. Plus tax.  - or can use miles (bus was 10k so assume @ 100/$1 for any)
Alaska Mileage Plan- Does not issue infant tickets.
American - 10% of full fare
British Airways - Taxes and 10% of miles
Lufthansa/Miles & More - Taxes (Usually $17.50)
United - 10% of full fare
US Airways - 10% of full fare
singapore- 10% of full fare (when flying Sq you could find a date showing a discounted fare)
ANA?
IB - 10% Full fare
Asiana- 10% of the adult mileage ticket, plus taxes & fees

if your ticket is not noted change the BD then call back for the bassinet

They will take care of you at the airport when you check in. Free.
If you want, you can call AA now and they may note it.

just called singapore to book EWR-MBJ on a UA flight on miles with an infant and they told me it could not be booked and i would just have to book a "commercial ticket" with united myself. i dont know if that means i can then add just call united and add an infant or if i would have to actually book a new ticket for the baby?

On the off chance that anyone has any experience with this..

I'm assuming that all travel as a lap infant must be completed before turning 2, meaning I can't book a flight on Dec 31 that lands Jan 1 when the child's birthday is Jan 1. What if I book it for Dec 30, with all travel to be completed by Dec 31, and there's a delay causing the trip to extend onto the 1st, when the child will be 2? I realize this is a long shot but I'm looking to do exactly this, so if there's a potential issue I'll give myself more space. It's hopefully going to be an LH F award booked with MM.

do you have any link where it says that you have to land in your final destination before the infant turns 2?

I would guess that as well, but what if the cabin is full? And worst case, if they tell me I need to book a new seat for her.

Why not just book him/her a seat? Why do you need to change the age in order to do that?
