1. Where should I call to cancel, I had a hard time when I called in the local clinic that I used to use.
I moved back a few years ago and at the time I cancelled by emailing a cancellation form to TouristJer@meuhedet.co.il with a request for them to confirm once cancelled, and I got a response saying בוטל on the same day, but warning that it could take up to a month to actually cut off and stop charging my card (go figure). PM me your email address if you want a copy of the form.
Obviously things may have changed in the past 3 years though. Probably worth the 5 min that it would take to fill out the form though.

f you are a Meuhedet member, cancellation can be done:
In person – visit your local clinic and sign a form stating that you wish to stop receiving Meuhedet services.
Via fax – submit a letter that lists your name, your Meuhedet ID number, the current date and the date of your departure. Explain that you wish to stop receiving services. Fax number: 02-531-2778. If you do not have Meuhedet, contact your kupat cholim to confirm that they accept fax requests.
Note that if you are a member of Bituach Leumi, your kupat cholim membership will stop as soon as your Bituach Leumi file is closed. However, you should cancel your supplemental insurance plan separately, if you have one (e.g. Maccabi Sheli, Meuhedet Adif/C).
