There are plenty of free polling tools which easily allow for conditional questions. I think these are vital:

I was previously infected with Covid
If yes:
With a positive PCR or antibodies test
Based on symptoms without a confirming test.
Agree, those would be needed for thorough survey.
Disagree, that those should be included in initial survey.   Rather wait for first results, then create survey for whichever category is of interest, eg,  if the "definitely not" category is huge, a followup might ask about the different reasons for being anti, where "prior confirmed covid" and "prior suspected covid" are some of the choices.

Also need to ask about "exposure". Saw someone mention that today.
Yes, all those and more important for understanding the situation.  But the more complicated the questions, the fewer people will complete the poll and the longer time it will take to analyze the results.

And there was a survey a few months back, allegedly sent by Hatzalah?  Did that include questions about the vaccine?  I think the results were mentioned here.   @TimT

With due respect, you are simply wrong.

I know of people who got vaccinated last week. Someone they knew tested positive, so they got the shot. I had a discussion with people mad at a 70 year old anti vaxxer for not taking the shot, but they themselves haven't gotten vaccinated. Plenty of people have questions and are unsure, but not staunchly anti vax. The biggest concern, IMHO is fertility. There are very convincing campaigns on social media that Israel is hiding an epidemic of vaccine-induced infertility. Even people who have gotten the shot believe in the cover up.
And for men, I think the main hesitancy is natural immunity rendering it moot (even if they aren't sure if they had covid! Go figure)

Overall, the issue is people just don't trust public health officials, and believe doctors might be shills, naive, or afraid to speak up. So they aren't sure whether to believe the conspiracy theories or not.

I could be wrong. Wouldn't be the first time, and won't be the last. My personal opinion, based on what I've been seeing, is that people's minds aren't changing, and especially not because of arguments online. They may have been skeptical, or procrastinating, and the situation on the ground pushed them to take the shot. But I don't think anyone is getting convinced that the shot is now safe if they doubted it before.

Thanks!  :)
I wonder if they've done a followup survey.
I remember now, there were huge differences between chassidish, yeshivish, modern orthodox.

Hope it isn’t too late... Some of those lined up for their first shot could get and spread COVID before they even hit the 3-4 weeks for their second dose.
